New born birth certificate for residents in Japan and Abroad – Married parents

NEW BORN BIRTH CERTIFICATE FOR RESIDENTS IN JAPAN AND ABROAD (Married parents)

IMPORTANT

  • No appointment for application.  
  • Those who come will not be received.
  • The Embassy accepts documents and fees by postal mail only .
  • Application processing time: 4 weeks opening days.
  • BEFORE SENDING DOCUMENTS, kindly email a reachable telephone number to the Embassy. 

FIRST STEP

  1. Go to the City Hall for issuance of :
  2. Legalized copy of the “Birth Notification & Birth Certificate”(出生届・出生証明書) you have submitted to them just after the birth. The legalized copy document is called “shusho-todoke-no kisaijiko”(出生届の記載事項) ;
  3. Certificate of Acceptance of Birth (“shusho-juri-shomeisho” – 出生受理証明書);
  4. Certificate of Residence (“juminhyo” – 住民票).

2.Translation of Japanese documents:

The document “Birth Notification & Birth Certificate” have to be translated by one of recommended translation companies below.

REQUIRED DOCUMENTS

For a child born in Japan

Legalized copy of the “Birth Notification & Birth Certificate” form, that you have submitted to them just after the birth, called “shusho-todoke-no kisaijiko” (出生届の記載事項). 

Attention:  do not send a photocopy of a Birth Notification form without city officers’ seals ; the Embassy needs a legalized document issued at the municipality or the local Legal Affairs Bureau.

For a child born abroad:  

  • Medical Birth Certificate issued at the hospital and/or Birth Certificate issued at the Civil Registry (Civil Registration Centre) ; in case it is in a foreign language, 
  • Translation in English or French by an approved translation company ;
  • Parents’ documents:
  • Original of Cameroonian Marriage Certificate of the parents;
  • Original of Passports of the parents;
  • Original of Cameroon National Identity Card if possible (if available, even expired), 
  • Both sides colour copy on one page of Residence Card of parents;
  • Original of Birth Certificate:
  • Cameroonian parent(s): original is compulsory. In case you don’t have it with you, ask to a relative in Cameroon to send it to you by DHL. If he/she cannot find it, ask him/her to go to the High Court and/or the Civil Status Registry for an issuance of a DUPLICATE with a Certificate of Strain Existence.
  • Non Cameroonian parent: original is compulsory. In case his/her Birth Certificate (or Family Register) is in foreign language, its translation by an approved translation company;

  • 1 more Letter Pack for returning original documents before the issuance of the requested document (if necessary; in that case, mention it in the application letter).

□ For residents abroad

About bank transfer:

  • Fees for the document and for its expedition can be sent by bank transfer, by adding bank charge that can’t be determined in advance. In that case, be aware that you have to add 6,000 JPY to 8,000 JPY, depending on the amount you are sending and the rate of the day. Please refer to your local bank.
  • If you have an acquaintance residing in Japan, he can send by postal mail by registered mail for cash.

About returning fees :

  • EMS fees from Japan to Australia and New-Zealand: 3,150 JPY

(rate can be changed anytime. Please refer to the website of the Post)

  • DHL approximate fees from Japan to: Australia and New-Zealand: 16,000JPY

(rate can be changed anytime. Please refer to the website of the DHL).

  • If you have already an account number especially for collect (Payer Account Number), don’t forget to mention it on your DHL airway bill.
